PRE-EMPLOYMENT
Drug Testing will include a
"5" panel test (10 panel available):
Marijuana
Cocaine
PCP
Opiates
Amphetamines
If the "donor"
(-applicant-) tests positive ---------> the laboratory will run a GC-MS (gas
chromatography - mass spectrometry) confirmation, which is virtually 100% accurate, and
upheld by the Supreme Court.
LAB ---------> MRO (medical
review officer) ---------> DONOR (mro has 5 days to contact the donor ) --------->
MRO will notify the employer ---------> Employer must in writing notify the employee
within 5 days.
GUIDELINES
1. all applicants for
employment must be tested
2. you cannot hire anyone
who tests "positive"
3. the applicant may
reapply in 3-6 months
4. all ads for employment
must state that you are "Drug Free
Workplace"
5. applicants must review
"summary of the policy"
6. all applicants must
sign a consent form
7. all applicants must
sign an acknowledgment of "Drug Free
Workplace"
8. test results must be
filed separately and securely (considered
medical records). Results are extremely confidential
9. sign must be posted
"DFW" visible to all applicants and customers
10. it is our recommendation
that drug testing on applicants for
employment be conducted no later than the following morning
after the interview
POST ACCIDENT
You must notify all current
employees that you are a "Drug Free Workplace" sixty (60) days before you may
test.
If during the sixty 60 days an
employee states that they have a problem or anytime thereafter and they haven't been
selected for random testing, they:
1. cannot be terminated
2. must be placed in a
treatment or recovery program
3. and up to 2 years must
undergo some follow-up testing
All reportable accidents
require drug testing:
1. stabilize the victim
(employee/patient)
2. if they cannot go to a
collection site, someone will come to them
3. testing must be implemented
within 36 hours of the accident
REASONABLE SUSPICION
1. Signs of impairment
2. Document in writing
reason for the testing (employees
performance) within seven (7) days of suspicion. Document any
pattern developing:
